1. Client Intake Form (Online Submission)

Start here! This required form gathers essential information about your pup’s health, behavior, and preferences. It also includes all policies and disclosures. Please fill this out after your Sniff & Greet Consultation so we can tailor the visit to your dog’s unique needs.

Download this version if you’d rather complete the intake by hand or just want to review everything in detail before submitting online. You can print it out, fill it in at your own pace, and either:
